Request for Information

General Information

Requests for Written Verification of a License or Registration: Pursuant to Iowa Administrative Code 650 - 15.4(2), the cost for written verification of a license or registration is $25.00. Requests for written verification or certification will not be processed until the fee has been received by board staff. Please specify in your request where the verification or license or registration needs to be mailed.

Payment of Fees to Iowa Dental Board: Please submit all payments in the form of a check or money order made payable to the Iowa Dental Board. The Iowa Dental Board is unable to process credit cards or debit cards for payment. Do NOT send cash for payment of fees! The Iowa Dental Board will not process requests until the appropriate fee has been received by the board office.

Examination Results: Examination results are confidential; therefore the Iowa Dental Board is unable to release examination results over the phone, via fax, or email. Examination results will be mailed to the address provided on the answer sheet. Please allow seven business days for results to be mailed.

The Iowa Dental Board established the Iowa Practitioner Review Committee to evaluate, assist, and monitor the recovery or rehabilitation of dentists, dental hygienists, and dental assistants who self-report possible impairments. To find out more, go here.
